We provide standard pricing and payment occurs at the time of service – no surprise medical bills.
The Osteopathic Clinic of Northern Colorado does not bill insurance and is out-of-network, including Medicare insurances.
At each visit, a document called a Superbill can be provided. These are essentially medical receipts that a patient can submit to insurance for reimbursement. As always, each insurance plan is different and coverage varies. Since Dr Anthony DO is a physician, these visits should be considered a doctor’s visit. Superbills will also be accessible on the patient portal.
With few exceptions, the medical billing CPT codes used are:
New patient visit: 99205
Follow-up visit: 99214 or 99215
Payment can be made at time-of-service with cash, credit card, or personal check (there is a service charge for return checks). You can also take advantage of your Health Savings Account (HSA) or Flexible Spending Account (FSA).
We accept patients with Medicare insurance, but the cost of a visit is legally required to be paid fully out-of-pocket and not billed to Medicare. Therefore at the initial visit, there will be a form to sign with specifics for any patient with Medicare insurance.
Unfortunately, we are also legally prohibited from seeing patients with Medicaid. Please see the Medicad policy below:
Adult initial visit – $300
Adult follow-up visit – $195
Pediatric (<18yo) initial visit – $240
Pediatric (<18yo) follow-up visit – $150
OB visit – $195
OB postpartum mom & baby visit special – $150 (if seen 3 or more times during pregnancy)
If Dr Anthony had been seeing you regularly at a previous clinic within the past year, please schedule your appointment as “follow-up visit”
